Serious case reviews are carried out when abuse and neglect are known or suspected factors when a child dies (or is seriously injured or harmed), and there are lessons to be learnt about inter-agency working to protect children. At least every two years, an overview analysis of serious case reviews in England is conducted to draw out themes and trends, so that lessons learnt from these cases as a whole can inform both policy and practice.

The third in the series of biennial reviews covers the period 2003-5 and looks at using the learning from serious case reviews to improve multi-agency practice at all levels of intervention including universal services and early intervention and to analyse the inter-acting risk factors for the children involved.

The methodology consisted of textual and numerical analysis of a near-complete sample of 161 cases of which 47 were analysed in more detail. An analysis of inter-acting risk factors for affected children formed part of the work.
