The Aiming High for Disabled Children programme is seeking to improve the transition process for young disabled people moving to adult life. This resource contains information, advice and guidance for practitioners at all levels seeking to improve transition support for disabled young people and their families.
Effective transition for disabled young people requires early planning to ensure that services provide a continuity of support that is focused on individual need. Disabled young people should be fully consulted in accessing their preferred services.
Difficulties have persisted with transition to adulthood as services have had differing criteria for access and differing transition points. Children's trusts are now bringing together services for all children.
Joint commissioning, underpinned by pooled resources, will ensure that those best able to provide the right packages of services can do so. Children's trusts are able to provide services to disabled young people up to the age of 25.
These resources will enable professionals to understand the transition process and will support the delivery of the forthcoming Transition Support programme.
